Is medical history centrally recorded -and accessible by various clinics?-

I'm wondering:

 

What happens to your data when you visit a doctor as a private patient?

 

You do show your ID usually, but do your test results, diagnoses and/or prescriptions get recorded?

 

Probably yes at the clinic level -at least the better-organized ones where you become a patient-.

 

But is there some kind of general registry that other clinics may be able to access?

Currently there is no central storage (yet, it's being discussed though), but your doctor/hospital has to store your medical records for 30 years.
