You can't test every single material that you use in a product when the materials and products are so complex, and it is more likely that they tested at the beginning and that over time the quality was "adjusted" to save costs and then never picked up.  Remember also that NASA don't make most of the components they only assemble them.  So an incoming component is already extremely complex and full of different materials pre-assembled, and when such components costs 10s or 100s of thousands of Dollars then you are less willing to take them apart to test every little bit, they just test the whole part functionally. 

 

I guess that you need to have some level of trust.  The the purpose of having a certification process is that you put processes in place to ensure that the quality should be good enough.  But obviously in this case the process was not enough and hopefully now they will resolve the issue.

 

You will always get people who commit fraud, and even if NASA did have their checks all the time, it would have still been possible for the company to bypass the checks by paying somebody off, so

Not really a complete fail, a large diesel generator powering an electric charger is still much more efficient that a normal diesel car.
