How to format a translated "Führungszeugnis"

I just received my Fuehrungszeugnis in the mail that I need for Australian immigration. I need to submit a translation of it, and the guidelines DO NOT require (but recommend) a professional translation. The Fuehrungszeugnis is a very simple document, and most of the translation is just name, address, date of birth, etc. I want to just do it myself, because my German is quite sufficient for this. The thing I'm unsure about is how to format the translation. Do I try to make it look somewhat like the original? Is it sufficient to just write it in a letter form? Can someone please tell me how a translation is usually formatted?

Follow the format of the original as closely as possible so the reader can orient him/herself. If the language is simple enough then translate it yourself, add an affidavit that you are in command of both languages and sign that affidavit in the presence of a notary who then notarizes your signature. I do this for my law firm all the time and have never had problems with authorities.

Thanks. I actually don't have to have anything notarized according to the documentation regarding translations...and if you can believe this, I emailed my immigration case worker, and he emailed back within FIVE MINUTES (!) and said it would be fine if I translated it myself. Aussie, Aussie, Aussie!
