When I'm in Germany, I use my internet phone card at www.idphonecard.com.
You don't have a card, per se, just a pin. You dial the number and enter your pin just like a regular "card'.
It's 8 cents a min from Germany to the US with no connection fees or monthly fees or expiration.
Actually, I've used them for about 4 years now and at home in the US for my all of my long distance.

You do know that you can phone the USA for as little as 1,39€ cents a minute using a call-by-call number from your land line don't you?

With T-Home (Deutsche Telekom), you can pay 3.99 euros a month to have unlimited free calls to the USA and most of Europe.

Obviously it depends how much you phone, but at 1,39 cents you could phone for nearly 5 hours a month to the States for $3,99.

I buy my US calling cards from the Indian shop Kohinoor @ Adolf Koplingstrasse behind
Mathaser in town. The card says 5 Euros but the shop takes 3.50 euros and gives me about 300+ mins to the US. I have been using these cards for 5 years now and they are better than a prefix code coz sometimes these companies tend to send you a bill for services that you never got.
I've used the prefix services for years and was never charged for anything I didn't get. If I were, I'd just contest it.

I use a VoIP provider called www.voipcheap.com. They charge 12.50 Euros (Payable with credit card) for 3 months and you get 300 minutes per week (Land and mobile both). Well you can call almost all of europe for the same offer.
The downside is that you need
1. A DSL Connection
2. A Computer to install their client or a VoIP Adapter (I paid 50 Euros for mine)
I've been using their services for the past two years and I must admit I've never had problems and the sound quality is as good as normal Telephone line.

I have DSL and ISDN from MNet.
http://www.m-net.de/ 31.90 Euro for ISDN + DSL
They offer a calling plan (no VoIP) essentially for 2 ISDN calls simultaneously "Topländer-Flat" which includes the US and Canada and Europe for an additional fee of 6.90 Euro.
