Women do not get exceptions. Men can carry a small bag too, or a camera bag, or a laptop bag. Most airlines do allow two pieces (of which one has to be a camera bag, a laptop bag or a purse etc.).
Exceptions may be the cheapo airlines, I think Clickair (who I just flew with today, but I only had one carry on piece) also only allows one. LH and the star alliance airlines I have flown with so far allow two.
I personally am glad they do not enforce them so strictly, because some people are not just going away for a weekend, some are going away for several months and have to squeeze everything into one suitcase of 20kg and one carry-on. I do usually comply with the size limits, just not the weight limits. Spain Air allows a carry on of 6kg. My trolley is 3kg without anything in it, I think it ended up being 12kg or so when I went home for Christmas. Luckily most allow more and carry-on bags are hardly ever weighed. I also usually pack around 2kg more in my checked bag (lately, with flying back and forth between Spain and Germany, I have been having a lot of baggage because I already took back some stuff, like books I have read, university stuff, and brought back new books to read). I never had to pay for extra luggage.
When I moved back to Germany from the States, I technically had four carry-on pieces, a regular carry-on bag, a laptop bag, a purse and a pillow (in a bag). No one said anything.
