Telemarketing - anyone have experience?

I've used quite a few Companies for telemarketing campaigns but most have a tier costing structure.
Instead I'm looking for flat pricing i.e just "per appointment" costs...anyone have any experience in this area? Looking for a starting place based on the below.

Projects are fairly simple, arranging B2B appointments with the Management of small companies of whom no previous relationship exists. Products have leading brand names and generate good interest from the market as a more reliable Industrial replacement product. In most cases the prospect base would already be mail droped.

In addition- If there are native resources (or as good as) out there in German, French, Dutch or English who can prove professional conversion rates and are interested in generating a quote, drop me a PM for more info. You need to have your own infrastructure and be invoiceable.
