The Standesamt is where you would get married should you choose to do so in Munich. But your post sounds like you are doing this elsewhere (like I did) sooo...
The Einwohnermeldeamt in Munich proper is the
Kreisverwaltungsreferat which is down at
Poccistr. First, I had to register the new name with the Burgerbüro (on the ground floor). This is where you get your Anmeldung and Lohnsteuerkarte. I had to produce the official Trauschein (marriage certificate). They would not accept the document provided at the ceremony. My marriage certificate took almost a year to get (blame Canada!) so I had to wait this long to make the name change.
Once the change is made in the computer, if you need to, proceed to the Auslanderbehorde (one floor up). Then you can get a new Aufenthaltserlaubnis (residence permit) if you need it. You go to the letter of your new name (example I changed from 'P' to 'F' so I had to get new paperwork from 'F').
If you need to change your name on your German driver's license, you need to take passport (showing married name), passport photo and 24 euros to the Kraftfahrzeugzulassungsstelle which is on Eichstätterstr. (near
Westendstr.) and apply for the change. In 3 weeks (so they say) you will get a letter advising you to pick up the new license and exchange the old one in.
