How to settle up after having left Germany

So I'm an idiot and forgot to cancel my bahncard before they sent me a new one. Now I need to pay up for my mistake. They sent me an Ueberweisung form to fill out (which I did before). The form was rejected even though I filled out all information. How should I proceed?

I should clarify myself here (the Ueberweisung is attached)...

A wire transfer from my bank to an international account is $45 and I only need to pay 55Euro for this! Besides Western Union, I don't see a suitable alternative. I tried contacting them via email and received a formulaic response.

Have you asked to see if DB will just cancel your BahnCard and forget the whole thing?

XeTrade/Custom House Currency Exchange is a possibility. Sending checks is free; a wire will cost ca. 12EUR.

(Such an Überweisung-form is usually used by the customer/their bank and is NOT sent to the company requesting the money.)
